 Proxy Healthcare partners with community providers to deliver specialized wound care for patients who need focused, consistent treatment. We serve as an additional wound care resource — supporting existing plans of care without replacing primary providers or home health services.

Our role is to help improve access to wound expertise, reduce delays in treatment, and maintain clear communication throughout the healing process.

 We work alongside a wide range of community providers who support patients with chronic or complex wounds. Our services are designed to complement existing care and provide additional wound expertise when needed.

Common partners include home health agencies, skilled nursing and long-term care facilities, hospitals and discharge planners, primary care providers, podiatrists, vascular specialists, and case managers.
