No. We serve as a supplemental wound care resource and work alongside existing providers. Our role is to support—not replace—the current plan of care.
We typically request a patient facesheet, recent care notes (last 30 days), and any wound-specific documentation or testing. Additional details may be requested depending on the case.
Following patient visits, we provide clinical updates within 72 hours. Documentation is shared via fax or secure email, depending on provider preference and established communication workflows.
We may see patients in long-term care settings depending on the facility and referral circumstances. Some facilities have preferred wound care partners in place, which can affect whether outside wound care services are utilized.
For patients in skilled nursing facilities (SNFs), coverage and payment responsibilities differ from other care settings. In many cases, wound care services provided during a SNF stay are the responsibility of the patient or facility. Our team can help review the situation and determine whether services are appropriate and how care may be coordinated.
